HOW DO WE COLLECT INFORMATION FROM YOU?
We kindly ask you arrive 10 minutes prior to your first appointment, so you can complete a Medical History and Consent form. All consent forms will be required to be signed by both the client and skin specialist and stored securely. Personal details will be transferred over to the computer software.
WHAT TYPE OF INFORMATION IS COLLECTED FROM YOU?
Your basic contact details will be provided including Full Name, DOB, Mobile number and address. You will also fill out a health questionnaire upon your appointment. During your booking we will require your card details, these will be held securely in the cloud and only used if you fail to make your appointment or cancel within 48 hours notice at which point a 50% cancellation fee will be redeemed.
HOW IS YOUR INFORMATION USED?
The information we collect about you is used to ensure we provide you with the best and most appropriate products and services. We will remind you of future booked appointments and also send information about promotions on treatments and products, that we believe would be of interest to you.
We use your contact information to respond to queries from you. We may also occasionally contact you to ask for your feedback on services we have provided.
WHO HAS ACCESS TO YOUR INFORMATION?
Only the clinic. None of your information will be passed onto third-party’s or anyone business.
STORAGE, PROCESSING AND RETENTION OF INFORMATION
We retain your information for as long as reasonably necessary to provide our products and services and to maintain records to satisfy HMRC, tax and other legal requirements.
HOW AND WHEN WE MAY SHARE YOUR PERSONAL INFORMATION
On rare occasions and only with your permission we may disclose information to our brands such as Medik8, Skinbase & Heliocare if you have an issue or problem. If a claim has been made, we would forward relevant details to insurers/police if required.

When you complete your consultation form, please indicate if you are happy for your photos to be used on our Social Media & Website Platforms. If you would like to approve the pictures prior to being used, please just say. If you indicate you are not interested then we will not share any images without prior consent.
